homework (amanda & arun) & apologies

apologies for not seeing you at the end of the day today - got caught up in finals-related planning

as i remember we had to update on:

- causes of aortic regurgitation (in a memorable & useful way)
- how not to kill people with a sickle chest crisis

  1. Causes of aortic regurgitation:

    To begin we can divide the causes into 'valvular' and 'aortic root' pathologies.

    Valvular Disease: CRIP

    C - congenital bicuspid
    R - rheumatological (rheumatoid arthritis, SLE)
    I - infective endocarditis
    P - pharmacologic agents e.g. fenfluramine, phentermine (appetite suppressants)

    Aortic Root Disease: SHAMOS

    S - seronegative arthritides (ankylosing spondylitis, Reiter's syndrome, psoriatic arthritis)

    H - hypertension
    A - aortic dissection
    M - Marfan's disease
    O - osteogenesis imperfecta
    S - syphilis

    what are your sources?

    there is a difference between ank spond & reiters - 10% of patients with ank spond will develop AR given 30years; incidence of AR in reiters is much, much lower (palazzi et al 2008 for ank spond data)

    would maybe add trauma - rare but important (CRIPT)
